Check Eligibility

Step 1:In the Member ID box, type the Member ID.

Note:If you do not know the Member ID, click the Lookup Member ID link to find a member using the member's last name, first name and date of birth.Then, continue onto Step 2.

Step 2: Click the Show Eligibility Copay button.

The member's Eligibility and Copay history are displayed in the bottom part of the screen.
