Dear Provider,

WellCare has completed a review of its mapping to HIPAA Claims adjustment reason

codes to better align with recent updates. Please note that the Plan will continue to

periodically publish changes to this list of codes to remain current with industry

guidelines. The first of this update series of HIPAA Claims Adjustment Reason Codes

for the Plan will become effective on October 24, 2008.

We request you observe these changes as they are likely to affect your billing

reconciliation process and/or systems. Our updated list is available to you via our Web

site at www.wellcare.com under Provider Resources. Your local Provider Relations

representative or our Provider Services department may assist you in obtaining a copy.
